🚨 Staff Complaint Form

Do you believe a staff member has acted unfairly, abused their power, or broken the rules?
This form allows you to report your concerns respectfully and confidentially. 🕵️‍♂️
All complaints are reviewed by the Head Admin or higher ranks only.

⚠️ Important Rules Before Submitting

• 
  
• 🧠 Be honest and respectful – false reports will not be tolerated.
  
• 📂 You must have (screenshots, logs, clips, etc.)
  
• 🕒 Complaints older than 7 days may not be reviewed.
  
• 🚫 Do not argue with staff – report it here instead.
  

📝 Staff Complaint Template

Use this structure when submitting your report:

2️⃣ Accused Staff Member

3️⃣ Description of the Situation

• 
  
• Clearly explain what happened. Include as many details as possible.
  
• When and where did this happen? (e.g., during a sit, admin chat, etc.)
  
• What rule or behavior do you believe they broke?
  

4️⃣ Evidence

• 
  
• 📷 Screenshots, 📹 video clips, or 📄 logs supporting your claim.
  
• Include links (e.g., imgur, medal.tv, YouTube) if possible.
  

5️⃣ Desired Outcome

• 
  
• What action do you believe should be taken? (e.g., warning, investigation, apology)
  

✅ What Happens Next

• 
  
• 👀 Your report will be reviewed by Head Admin+ only.
  
• 🗣️ You may be contacted via Discord for more details.
  
• 🔒 Everything remains confidential — only management can see complaints.
  

🚫 Do Not Use This Form For

• 
  
• Player reports (use the Player Report form instead).
  
• Minor disagreements with staff decisions.
  
• Ban appeals (use the Ban Appeal form).
